Dear Parent/Guardian:

Welcome to Galesburg-Augusta Community Schools. As a parent/guardian of a new or transfer student, we hope your transition is a smooth and positive experience. With your help we plan to maintain a safe and healthy environment for your child to learn and grow. All enrollment is done online or at the school offices. Please contact the school your child will attend to enroll.

Emergency Card Notice

It is very important that the emergency card is completely filled out with a designated person authorized to pick up your child in the event we cannot immediately reach you. In addition to accident or injury, it is important that you have a designated person or network available for your child in cases of early school closings or inclement weather.

If your child has a medical condition i.e., diabetes, epilepsy, asthma, acute allergies (food, insect bites, animals), or physical disability, please inform the teacher or school secretary to insure a safe, healthy school environment.

Kindergarten Age Requirements and Waiver Request

Michigan law amended the Revised School Code (MCL 380.1147), which changed the minimum age requirement for a child enrolling in kindergarten. Beginning with the 2015-2016 school year, a child who resides in the school district may enroll in kindergarten if the child is at least 5 years of age on September 1 of the school year of enrollment.

  • For the 2015‐2016 school year (and beyond), children must be five years old by September. 1st.

Parent Waiver Option

If the child is not eligible for enrollment per the guidelines specified above and will be five years old by December 1st, the child’s parent or legal guardian can apply for a waiver to enroll their student in school. Waiver deadlines and other information are as follows:

  • If the child resides in the school district, the waiver must be submitted to the school district no later than June 1st before the beginning of the school year.
  • If the child becomes a resident of the school district after June 1st, the waiver must be submitted to the school district no later than August 1st before the beginning of the school year.
  • When in receipt of a waiver, districts can make recommendations to the child’s parent or legal guardian, as to the child’s readiness for school, and whether or not the child should be enrolled early.
  • Regardless of a district’s recommendation, the parent or legal guardian retains the sole discretion to enroll their child.

Health Requirements for Starting School

Prior to a child entering or attending school, parents or guardians are required to produce documentation confirming their child has received the school required immunizations, or in the alternative, their child received at least one dose of each of the required immunizations and is awaiting receipt of subsequent doses to be administered at appropriate intervals. In the following tables these children are considered “complete.”

Submit a Health Appraisal form completed by your physician. Must be completed by the first day of school. Minimum State of Michigan Immunization requirements:

All Kindergarteners & 4 – 6 year old transfer students:

  • 4 DPT – 1 after 4 years of age
  • 4 Polio – *Only 3 doses are required if dose 3 is administered after 4 years of age
  • 2 MMR – 1st after 12 months of age
  • 4 HIB – if entering before age 5 or entering the pre-school
  • 3 Hepatitis B – vaccines in series
  • 2 Chickenpox vaccine – after 1st birthday or date of history of the disease

All 7th graders & 7 – 18 year old transfer students:

  • 4 DPT – 1 dose Tdap at 11 yrs of age or older
  • 4 Polio - *Only 3 doses are required if dose 3 is administered after 4 years of age
  • 2 MMR – 1st after 12 months of age
  • 3 Hepatitis B (vaccine series)
  • 2 Varicella (chickenpox) - after 1st birthday or date of history of the disease
  • 1 Meningococcal for children 11 years of age or older upon entry into 7th grade or higher.

Each year schools are required to report the immunization status of children enrolled in kindergarten and 7th grade (6th grade prior to 2014) and new entrants to the school district. If you have a religious or medical reason why your child cannot be immunized, a waiver must be signed before starting the first day of school.

Medication (Prescribed & Over the Counter)

For the safety of your children, Galesburg-Augusta Community Schools has a medication policy which requires written instructions from the prescribing physician or health care professional and a written request from both a physician and one parent/guardian. The request and permission must be renewed each year.
